Scarless Under-Eye Surgery: How the Transconjunctival Approach Works

An incision inside the lower lid leaves no external scar and can still fully reposition fat. What transconjunctival access does well — and when a skin incision is unavoidable.

“Will I have a scar?” is the most common fear

For a lot of people considering under-eye surgery, the single biggest hesitation is not the operation itself but the mark it might leave. The under-eye is on constant display, and the idea of a visible line along it can outweigh the wish to fix a tired-looking bag. It is a reasonable fear, and it deserves a straight answer rather than reassurance-by-slogan. The honest version is this: for most under-eye fat problems, the surgery can be done through the inside of the lower eyelid, leaving no external skin scar at all — but there are specific situations where a skin incision is the right choice, and a clinic should tell you which of those you are before, not after, surgery.

What “transconjunctival” actually means

Transconjunctival simply describes where the incision is made: on the inside of the lower eyelid, through the conjunctiva — the moist inner lining — rather than through the outer skin. Working from behind the lid, the surgeon reaches the orbital fat and the tear-trough area from the inside. Because the whole approach happens on the inner surface, the outer skin of the lower eyelid is never cut. The word sounds technical, but the idea is intuitive: the door into the under-eye is placed where no one will ever see it, on the hidden inner side of the lid, instead of on the visible front.

Why there is no external skin scar

The reason the route is called scarless is straightforward: a scar forms where skin is incised and heals, and here the skin is never incised. The inner conjunctival lining is what is opened, and mucosal tissue like this heals quickly and does not leave a visible external mark. There is also usually nothing external to remove afterward, which means one fewer appointment and no lash-line healing line to watch. It is worth being precise, though: “no external scar” refers to the surface of your skin. It does not mean no surgery happened — real work is done inside — it means the evidence of it is not written on the part of the eyelid you see in the mirror.

It is not just fat removal — full repositioning is possible this way

A common misconception is that the inner-lid route can only be used to snip out a bit of fat. That is not the case. The transconjunctival approach can carry out full fat repositioning — the more sophisticated operation in which the tear-trough tether is released and the orbital fat is preserved and moved over the rim into the groove, rather than discarded. This is not a new idea: the transconjunctival repositioning technique was established in the surgical literature by Goldberg at UCLA in 2000, transposing orbital fat as a pedicle through an inner-lid incision. So the scarless route is not a lesser, removal-only option — it is fully compatible with the structural correction ST considers the better answer for most bags-and-groove patterns.

When a skin incision is genuinely needed

Honesty means naming the limits. The transconjunctival route does not tighten or remove excess skin, because it never touches the skin. When the main problem is significant skin laxity — loose, redundant lower-eyelid skin — or redundant muscle that needs to be addressed, an external approach such as a subciliary or partial-skin incision, placed discreetly just below the lash line, may be the correct choice. In those cases a small, well-planned external scar is the trade-off for actually solving the problem, and pretending the inner route can do everything would be dishonest. This is exactly why cause-based assessment comes first: the route should follow your anatomy, not a marketing promise that every case can be “scarless.”

Benefits beyond scarlessness — and its honest limits

Beyond leaving no external scar, the inner-lid route avoids cutting the skin and the muscle sling along the lash line, which is one of the factors associated with the lid being pulled down after aggressive skin-incision surgery. It also removes external stitch care from recovery. But the same feature that makes it scarless is also its limit: because it does not touch the skin, it cannot tighten loose skin or correct significant skin redundancy. It is an excellent tool for fat bulging, tear-trough tethering, and repositioning; it is not a skin-tightening operation. Matching the tool to the actual problem — rather than choosing “scarless” at the cost of leaving the real issue untreated — is the point.

Why it suits many Asian under-eyes

The inner-lid repositioning route is particularly well suited to a common pattern in younger Asian patients: a tear-trough groove with orbital fat protrusion, but skin that is still firm and not significantly loose. A published Japanese series of transconjunctival orbital fat repositioning in young Asian patients reported excellent results in the large majority and no major complications such as lid retraction, eversion, or double vision — evidence that, in the right anatomy, the scarless route delivers the structural correction without the risks associated with skin-incision surgery. It is not a promise of a specific number for you; it is a reason the approach fits many of the under-eyes ST sees, where skin quality is good and the problem is fat and groove rather than loose skin.

What to confirm in your consultation

Because “scarless” depends on your anatomy, the useful questions are specific. Ask whether your case is suitable for the transconjunctival route, or whether skin laxity means a skin incision is needed — and if so, where the incision would sit and how it is planned to be discreet. Ask whether the plan is fat repositioning or only fat removal, since the inner route can do either. If you have loose skin, ask directly how it will be addressed, because the inner route alone will not tighten it. Bring clear front, oblique, and side photos in natural light; they help the clinic explain honestly which route your under-eye actually calls for, rather than defaulting everyone to the same answer.
