Under-eye fat repositioning: what patients ask first.

It is a lower-eyelid procedure that moves protruding orbital fat over the orbital rim instead of simply removing it. The goal is to soften both the eye bag and the tear-trough shadow so the lower lid blends into the cheek more naturally.

A common reason is that the tear-trough ligament was not fully addressed or the repositioned fat was not fixed securely. If the groove remains tethered, fat removal alone can make the hollow or dark circle look worse rather than better.

Lower blepharoplasty is a broad category. ST’s focus is cause-based under-eye fat repositioning: preserve useful fat, release the groove, fix the fat pad, and add fine fat grafting only when there is a true volume deficit.

Sometimes they mask a hollow temporarily, but they can also create lumpiness, surface irregularity, or make revision harder. ST evaluates whether the problem is retained filler, over-removed fat, scar tethering, or an unreleased groove before recommending a plan.

Aegyo-sal is not the same as an eye bag. It is related to the skin and muscle that appear with expression. Fat repositioning can make the natural line clearer, but it is not a surgery to enlarge aegyo-sal or make both sides perfectly identical.
